Have the following eligibility/compliance information ready:

Application Form:

1. Download/print the Dean's Statement Form and have it completed and signed by an authorized person from your school's Dean's Office, Student Affairs, or Registrar’s Office
2. Have a copy of the Stanford clerkship contact's pre-approval, usually an email message saved as a PDF file. A pre-approval is required for each different clerkship being requested.

Acceptance Form & Medical Clearances (completed later if offered clerkship participation):

• Copy of your mask fit certification to show that you have successfully passed a Respiratory Mask Fit Test within the past 12 months (for Summer Quarter, no older than September 2022; for Autumn Quarter, no older than December 2022; for Winter Quarter, no older than April 2023; for Spring Quarter no older than June 2023)
• Enter health insurance policy details
• Provide proof of the following required immunizations and tests:
  • MMR (Measles, Mumps, Rubella): 2 doses of live vaccines or laboratory evidence of immunity
  • Varicella: 2 doses of live vaccines or laboratory evidence of immunity
  • Hepatitis B: 3 doses of vaccine, or laboratory evidence of immunity from vaccination
  • Tdap (Tetanus+Diptheria+Pertussis): Tdap booster (booster without Pertussis is not compliant), must be within past 10 years from the last day of the quarter
  • Flu vaccine: Required seasonally October thru March (flu shot for 2023-2024 flu season)
  • Tuberculin status:
    • TB testing completed within 12 months prior to end of clerkship, with laboratory evidence of negative QuantiFERON Gold or T-spot blood test. PPD is not accepted.
    • If history of a positive TB skin or blood test, a chest x-ray completed within 12 months of start of clerkship accompanied by positive TB test results.
  • Completion of COVID-19 vaccination plus a booster
• If applicable, Supplemental Liability Insurance policy if home school's insurance is below $1M primary and $3M aggregate

• A link to the application form will appear in the right side of this webpage if there's an active application acceptance period. Please complete the application form and indicate your clerkship choices in order of preference, including which period you prefer for each elective, if appropriate. Use the upload capability toward the end of the form to include required information. The application form must be received by the associated quarter's application deadline. 
• All visiting student applicants must use the current quarter's Visiting Student Application for Elective Clerkships Form, have a completed and signed Dean’s Statement Form, and have their Stanford clerkship pre-approval(s) obtained when applying.  
• Students will be scheduled on a first come first served basis, based on the application form submission date.

• At the time students are being scheduled and are accepting offered clerkships, instructions for paying the non-refundable $200 application fee will be sent from visiting-MD-students@stanford.edu.
• Payment of the application fee is required to complete the application process.

• If there is a clerkship available, visiting students will be sent an acceptance notification email by the notification date listed on the Clerkship Application & Period Dates webpage.  Waitlist notifications will be sent to those who were unable to be scheduled.
• The acceptance notification will include instructions for completing the Acceptance Form and for providing compliance information (immunizations, TB test, mask fit certification, and, if needed, supplemental liability insurance).  See Visiting Student Acceptance for more details.
• Follow the form's instructions for providing requested information and accepting the scheduled clerkship(s). Use the upload capability toward the end of the form to include required information. The Acceptance Form must be received by the deadline date listed on the acceptance notification.
• 
  The application process cannot be considered complete unless all eligibility requirements are met, the application fee is paid, and an Acceptance Form is received.

Some of the requirements are included on the Dean's Statement Form which must be completed and signed by authorized Dean's Office, Student Affairs, or Registrar's Office staff at your home school. Eligibility requirements are subject to change without notice.

Identify elective rotations of interest by reviewing the list of clerkships found in the Course Bulletin. Follow the visiting student instructions for obtaining the department's pre-approval provided in the course catalog's clerkship description.  Also review details of available elective clerkships and obtain required pre-approvals as listed on the Clerkship Descriptions & Pre-approvals webpage. A pre-approval is required for each elective clerkship that is selected on the application form.
